1. Services Provided

FORMHAVEN WORKS LLC provides user experience and interface planning services, including form experience design, dashboard interface planning, navigation and interaction mapping, and preparation of interface pattern documentation. Our services focus on the structure, usability, and visual organization of websites and software interfaces.

The Company does not provide software development, website hosting, payment processing, cybersecurity services, telecommunications services, or management of client information systems.

2. Scope of Work

All projects are defined by a written Proposal or Statement of Work ("SOW") agreed upon by both parties prior to commencement. Any changes to scope must be documented in a written amendment signed by both parties.

3. Client Responsibilities

The client agrees to: provide timely feedback and approvals; supply all necessary materials, access, and information required for the project; and designate a single point of contact with authority to make decisions.

4. Fees and Payment

Fees are as specified in the applicable SOW. Unless otherwise agreed, invoices are due within fourteen (14) days of issuance. Late payments accrue interest at 1.5% per month. We reserve the right to suspend work on overdue accounts.

5. Intellectual Property

Upon receipt of full payment, the client receives a non-exclusive, worldwide license to use the deliverables for their intended business purpose. FORMHAVEN WORKS LLC retains the right to display deliverables in its portfolio unless otherwise agreed in writing.

6. Confidentiality

Both parties agree to keep confidential all non-public information exchanged during the engagement and not to disclose it to third parties without prior written consent, except as required by law.

7. Limitation of Liability

To the maximum extent permitted by law, FORMHAVEN WORKS LLC's total liability for any claim arising out of or related to the services shall not exceed the fees paid by the client in the three (3) months preceding the claim. We are not liable for indirect, incidental, or consequential damages.

8. Termination

Either party may terminate the engagement with fourteen (14) days' written notice. Upon termination, the client is responsible for fees for all work completed to the date of termination.

9. Governing Law

These Terms are governed by the laws of the Commonwealth of Kentucky, USA. Any disputes shall be resolved in the courts of Bell County, Kentucky.

1. Nature of Services

FORMHAVEN WORKS LLC delivers professional interface planning and UX documentation services. Because our work involves significant time, research, and creative effort from the moment a project begins, our refund policy reflects the custom and intangible nature of these deliverables.

2. Deposits

Projects requiring a deposit (as specified in the SOW) are non-refundable. The deposit secures project time and covers initial planning, research, and onboarding activities.

3. Milestone-Based Refunds

For milestone-based projects:

Before work begins on a milestone: the portion of fees allocated to that milestone may be refunded upon written request.
After work has begun on a milestone: fees for that milestone are non-refundable.
After a milestone has been delivered and approved: no refund is available.

4. Cancellation by Client

If the client cancels the project after work has commenced, the client is responsible for fees corresponding to all work completed up to the cancellation date, calculated on a pro-rata basis against the total project fee.

5. Cancellation by FORMHAVEN WORKS LLC

If we cancel the project for reasons other than client breach, we will refund any fees paid for work not yet commenced.

6. Revision Requests

Refunds are not issued for deliverables with which the client is dissatisfied if those deliverables were produced in accordance with the approved SOW. We will work with you to address concerns through our standard revision process as outlined in the SOW.

7. How to Request a Refund

Submit refund requests in writing to support@formhavenworks.com within seven (7) days of the relevant milestone delivery or cancellation event. Include your project name, invoice number, and reason for the request. We will respond within five (5) business days.

8. Processing

Approved refunds are issued via the original payment method within ten (10) business days.
